Requesting provision (Wmo)

What is it?

If you need help, such as a wheelchair, customized transportation, assistance with housekeeping or an adjustment to your home, you can request this from the municipality. This is called: requesting a facility.

How does it work?

You notify the municipality that you need help. This can be done by going to the Wmo counter in person, but often also by sending an e-mail or a letter. At the Wmo counter, in the letter or in the e-mail, you explain what problems you have. For example: 'I can no longer cook for myself'.

Personal plan

Once you have reported your problem, the municipality will ask you to make a personal plan. You will send the plan to the municipality. In this personal plan, you will state what you think you need and why you need it. You have up to 7 days after the notification for this plan. This period starts after you submit the notification. If you do not submit a personal plan, there is nothing to worry about. It is not mandatory to submit a personal plan. It will not affect your subsequent application.

Research

As a result of your report (and possibly your personal plan), the municipality will investigate. The municipality will investigate whether you actually have limitations and whether others may be able to help you. During this investigation it will also become clear whether you will have to pay a contribution towards the costs of the facility.

When the investigation is complete, the municipality will provide you with the results of the investigation and solutions in writing.

Request

If it appears that there are no options other than applying for a facility, you can make an application. You may also make an application if you disagree with the other options the municipality offers you. So you may, after the investigation, always make an application.

What to do.

You report your problem to the municipality. You can do this in person, by letter or by e-mail. You may also call.

How long does it take?

The municipality must conduct the investigation within 6 weeks of your notification.

If you submitted an application after the investigation, the municipality must decide on it within 2 weeks.

If the municipality takes longer than 6 weeks to investigate, you may still submit an application after 6 weeks. You do not then have to wait for the investigation to be completed.
